The P/F Ratio mathematically quantifies how effectively the lungs are transferring oxygen from the air into the bloodstream, normalizing the patient's blood oxygen against the exact amount of oxygen they are being given.

The straightforward formula is: P/F Ratio = PaO2 (in mmHg) ÷ FiO2 (expressed as a decimal).

For example, a healthy person breathing normal room air (21% oxygen) with a PaO2 of 95 mmHg: P/F Ratio = 95 ÷ 0.21 = 452. A normal ratio is generally considered to be >400.

However, consider a critically ill pneumonia patient requiring 60% supplemental oxygen (FiO2 = 0.60) just to maintain a barely acceptable PaO2 of 60 mmHg. P/F Ratio = 60 ÷ 0.60 = 100. A ratio of 100 indicates severe lung failure and meets the criteria for severe ARDS. This ratio is far more diagnostically useful than looking at the PaO2 alone.
