Step 1 (Repetition): Instruct the patient: "I am going to say three words for you to remember. Please repeat the words after I have said all three." Clearly state: "Sock, Blue, Bed." Score 1 point for each word the patient successfully repeats on their first attempt (Maximum 3 points).
Step 2 (Temporal Orientation): Ask the patient three specific orientation questions: "What year is it right now?", "What month are we in right now?", and "What day of the week is today?" Score each question according to the precise grading rubric (Maximum 3 points).
Step 3 (Recall): Ask the patient: "Let's go back to an earlier question. What were those three words that I asked you to repeat?" Allow them time to answer without cues. If they struggle, provide category cues (e.g., "Something to wear" for Sock, "A color" for Blue, "A piece of furniture" for Bed). Score each word based on how much cueing was required to achieve recall (Maximum 9 points).
